Maxillary Left First Molar (UL6) Root Canal Retreatment – Fractured Instrument

Patient referred as root canal treatment had been started, following irreversible pulpitis, but a file had been fractured in the disto-buccal (DB) canal.

The separated file was initially bypassed and later removed with ultrasonics. The four canals were found and chemomechanically debrided. Obturation was with warm vertical compaction of gutta-percha and sealer. An amalgam core was placed in the access cavity. The prognosis for the tooth is good. The referring dentist was advised to place a cuspal coverage restoration.
